Max 2 cycles (4 transfers) for ages 23-39; 1 cycle (2 transfers) for ages 40-41. Same-sex couples must self-fund 6 IUI cycles. Male BMI must be < 35. Neither partner may be sterilised. [ICSI: Funded under same criteria as IVF if sperm quality/quantity is poor, zero sperm in semen but retrievable, or previous IVF resulted in poor/no fertilisation.]
